Scheduling
If
you have an urgent problem, contact our staff and you will be
scheduled for an appointment as soon as possible.
If our office is running late, we can help you schedule another
appointment. If you are more than 15 minutes late, we may have
to reschedule your non-emergency visit.
We need a minimum of 12 hours notice to cancel a visit; you
mayuse the 24-hour answering serviceto advise us. If you miss
2visits without notification, a $40.00charge will be assessed
and is required to be paid prior to the next visit.
Insurance, Payments and Fee's
We
accept most insurance plans. Some plans require a referral from
your primary care physician for visits to our office.
Insurances such as Medicare have certain restrictions on the
conditions that can be treated and reimbursed. Under some company
policies the cutting of nails, corns, and calluses are only
covered if you have circulatory problems or diabetes. If you
are unsure about your plan's requirements, please call your
insurance company for assistance. Co-payments and deductibles
must be paid at the time of service. A billing charge will be
added for late payments.
There is a fee for records requested by your lawyer or insurance
company. These charges are not covered by insurance and must
be paid prior to release of the report or form.
Your First Visit
You
can help us by being prepared to answer these questions:
- When did your problem start and what were you doing at that time? Where does it hurt? When does it hurt?
- Do certain activities or shoes increase the pain?
Please arrive 15 minutes early for your first visit so you will
have enough time to complete our health history form. Bring
a list of medications you are taking, including dosage and how
often taken. Be sure to tell us if you have any medication allergies.
Please bring your insurance card with you. In Many cases we
can use the information on the card to submit necessary billing
forms for you.
After you complete the health history, our nurse will talk with
you about your foot or ankle problem. Then, the doctor will
review all the information you have provided and examine you.
If an x-ray is indicated, it can usually be done in our office
during your visit. If no further testing is necessary, we will
discuss diagnosis and treatment plan and begin treatment.
Follow-Up Visits
We
will carefully review how your treatment worked and any symptoms
you are still experiencing. Based on this information, we will
continue or change your treatment plan. If you are in any way
dissatisfied with how you were treated or with the outcome,
please tell us. We will do everything possible to correct the
problem.
One of our physicians is always on call. We will offer suggestions for treatment or ask that you go to an emergency room, or you will be seen the next business day.
Medications and Refills
Refill
requests should be made during office hours; after-hours requests
cannot be honored because we are unable to review your medical
record. We require an office visit before giving a refill order
if you have not been seen for six months. We require an office
visit for a pain medication refill order if you have not been
seen for two months.
